The gate that will not open

A form asks whether the drawings were updated. A workflow refuses to close until they are.

We gate a change so it cannot pass until the technical review is complete, the affected documents are identified and the updates are attached rather than promised. Management of change reaches further than any other element, and it is the element paper handles worst.

    Routed to the disciplines the change actually touches, not to a standing committee

  3. Authorise

    At the level the risk rank demands, with the rank recorded

  4. Implement

    Work executed with the document impact list open and visible

  5. Update records

    Closure blocked while any drawing or procedure update remains outstanding

    Where a change can close before the record is updated, the update becomes a follow on action, it ages, and the next study inherits a drawing that is wrong.

    Verified before hazardous material returns, as a condition and not a follow on

Temporary means temporary

Temporary changes carry an expiry date that escalates on its own once it passes. A bypass therefore becomes permanent only by a decision somebody signs, never by the date going unnoticed.

Gates that hold will slow you at first

Enforced gates slow the first month. Engineers used to closing a change and updating the drawing later will find the workflow will not let them, and some of that friction is the point.
