Measurement and Metrics is the indicator system that lets your leadership see the operating state of process safety before it manifests as a catastrophic event. The element maps to API RP 754 with its Tier 1 to Tier 4 process safety event indicators, to CCPS RBPS Element 18 and to the leading and lagging indicator frameworks that emerged after Texas City. Tier 1 captures the worst events, a loss of primary containment with consequence, Tier 2 captures lesser losses of containment, Tier 3 captures challenges to the safety system and Tier 4 captures operating discipline failures. Without leading indicators leadership sees only what has already failed, and with them the trajectory becomes visible early enough to act. ICCA and IOGP publish parallel frameworks, and corporate HSE governance typically requires Tier 1 and Tier 2 reporting upstream.
