Operational Readiness and the Pre Startup Safety Review (RBPS Element 14)
The pre startup safety review that closes the gap between construction completion and live operation
Operational Readiness and the Pre Startup
Safety Review (RBPS Element 14)
Operational Readiness, delivered in practice through the pre startup safety review, is the final structured check before highly hazardous chemicals are introduced into the process. The element maps to OSHA 1910.119(i) and verifies that construction and installation match the design, that every PSM element relevant to the new or modified process is in place, that PHA recommendations have been closed or formally risk accepted and that operators have been trained on the updated procedures. CSB and HSE investigations of incidents after modification repeatedly cite a bypassed review, or a review without substance, as a contributing cause. The CSB reports on Williams Olefins, Tesoro Anacortes and BP Texas City all carried pre startup safety review findings.

How the study is executed
A structured, facilitated process that runs from scope definition through close out and produces defensible, actionable outputs.
We define the scope of the pre startup safety review to OSHA PSM (i) across new construction, a modification driven by management of change and a restart after a turnaround, aligned with your corporate review procedure.
We run a multi discipline walk down across process, mechanical, electrical, instrumentation and HSE, verify that construction matches the P&ID and document the deviations and their resolution.
We confirm that every PHA, HAZOP, LOPA and SIL action related to the change is closed or formally risk accepted with documented evidence, aligned with the PHA register.
We verify that operating procedures are updated to reflect the change, that operator training is complete with documented competency and that the emergency response procedure is aligned.
We obtain mechanical integrity signoff with relief sized and the vessel coded, confirm the safety instrumented system is commissioned through factory and site acceptance testing, issue the area classification drawings and commission the emergency shutdown and fire and gas systems.
We obtain signoff from your site leader and HSE manager before chemicals are introduced and specify the authorisation hierarchy and audit trail.

What the study covers in full
Outcomes of Operational Readiness and the Pre Startup Safety Review (RBPS Element 14)
- Catches the pattern of incidents that follow modification
- Verifies that PHA actions are closed before live operation
- Confirms operator training and that procedures are current
- A final gate before highly hazardous chemicals are introduced
- Evidence that withstands an OSHA PSM (i) audit
- CCPS RBPS Element 14 evidence
- Alignment with the CCPS Operational Readiness Guidelines
- The Factories Act 41B procedural requirements met
- A lower startup error rate
- A cleaner handover to operators
- Stronger discipline around close out of PHA actions
- Integration across every discipline
- An incident cost after startup avoided
- Less commissioning rework
- A strong position with insurers through the quality of the review
- Faster achievement of steady state
