Process safety culture is the set of shared values, beliefs and everyday behaviours that decides whether your formal management system genuinely keeps operations safe. Of the twenty RBPS elements this is the most foundational and the slowest to shift, and our team finds that almost every CSB major incident report names cultural drift somewhere in the root or contributing causes. We help your site leadership take real ownership of this element, sustain it through visible presence in the field and measure it with perception surveys scored against the Hudson maturity ladder that climbs through five stages from Pathological and Reactive into Calculative, Proactive and finally Generative. We quantify how much visible leadership there is in the field today in terms of hours per month, conversation quality and depth of near miss inquiry, then identify the gap against Proactive and Generative behaviour.
We codify the distinction between error, at risk and reckless behaviour following James Reason, design a proportionate response that ranges from coaching through counselling to discipline and train your leaders on the substitution test.
We design low barrier near miss and weak signal reporting channels, eliminate the patterns that drive retaliation, set supervisor acknowledgement within 24 hours and integrate the channels with the root cause analysis workflow.
We establish a leading indicator dashboard covering observation rate, reporting rate, leadership engagement hours and the corrective action close out cycle, and review it monthly with your site leadership.
We re assess the culture every 18 to 24 months against the baseline, track maturity progression and integrate it with your corporate ESG and process safety event indicator reporting.
