Human error appears in almost every incident sequence and in very few risk assessments with any rigour. This course covers task analysis, error identification and quantification, and the performance shaping factors that move an error probability by more than an order of magnitude.

Every module is taught with worked examples and exercises drawn from real plant, and adapted to your own process where the course runs in house.
How people actually fail
- Slips, lapses, mistakes and violations, and why the distinction changes the fix
- Skill, rule and knowledge based behaviour

- Why blaming the operator ends the investigation early
Task analysis
- Hierarchical task analysis on a safety critical task
- Identifying the steps where an error has a consequence
- Recovery opportunities and where they genuinely exist
Quantification
- Screening values and when they are adequate
- HEART, THERP and SPAR H approaches and their limits
- Performance shaping factors including time pressure, workload, alarm load and fatigue
- Dependency between successive actions by the same person

- Designing out the error rather than training around it
- Procedure and interface changes that move the number
