Phase 5 installs the designed safety instrumented system, commissions it through the factory acceptance test and the site acceptance test, and validates it from end to end to IEC 61511 clauses 13 through 15. Validation confirms that each safety instrumented function performs as the specification requires before any highly hazardous chemicals are introduced.

Phase 5 closes the design loop of Phases 3 and 4 and opens the operational loop of Phase 6. The validation evidence is also the foundation for the Stage 2 and Stage 3 functional safety assessment and for the pre startup safety review before any chemical is introduced.

Implementation methodology

How we implement this element

Factory Acceptance Test Development

We author a factory acceptance test that covers every cause and effect, every voting combination, every override scenario and the bypass logic for each function.

Factory Acceptance Test Execution and Witness

We execute at the vendor facility with a functional safety engineer or a TUV or exida witness and document the deviations and their resolution.

Installation Quality Verification

We verify that the site installation matches the design across cabling, segregation, redundancy and environmental ratings.

Site Acceptance Test Execution

We run the site acceptance test with end to end loop testing that verifies actuation all the way from the sensor through the logic to the final element.

Validation to Cl.15

We confirm that each function performs as the specification requires, we document the validation evidence to clause 15.2, and we align it with the pre startup safety review before highly hazardous chemicals are introduced.

Stage 3 Functional Safety Assessment

We carry out an independent Stage 3 assessment after installation and before operation, resolve the findings and hand over to the Phase 6 operations team.
