Installation, Commissioning, Validation
We run factory acceptance testing, site acceptance testing, loop testing and end to end validation to IEC 61511 clauses 13 through 15
What this element is and why it matters
Phase 5 installs the designed safety instrumented system, commissions it through the factory acceptance test and the site acceptance test, and validates it from end to end to IEC 61511 clauses 13 through 15. Validation confirms that each safety instrumented function performs as the specification requires before any highly hazardous chemicals are introduced.

Individual significance for organisations
Phase 5 catches the gap between what was designed and what was actually built. Facilities with rigorous validation discipline avoid the all too common pattern of a system that was commissioned but never tested from end to end and that only reveals its weaknesses during a real demand event, and our team makes sure your facility never falls into it.
Contribution to Functional Safety Lifecycle Implementation
Phase 5 closes the design loop of Phases 3 and 4 and opens the operational loop of Phase 6. The validation evidence is also the foundation for the Stage 2 and Stage 3 functional safety assessment and for the pre startup safety review before any chemical is introduced.
What compliant execution looks like
How we implement this element
A focused six step methodology calibrated to deliver installation, commissioning, validation as a working capability rather than a documented compliance artefact.
We author a factory acceptance test that covers every cause and effect, every voting combination, every override scenario and the bypass logic for each function.
We execute at the vendor facility with a functional safety engineer or a TUV or exida witness and document the deviations and their resolution.
We verify that the site installation matches the design across cabling, segregation, redundancy and environmental ratings.
We run the site acceptance test with end to end loop testing that verifies actuation all the way from the sensor through the logic to the final element.
We confirm that each function performs as the specification requires, we document the validation evidence to clause 15.2, and we align it with the pre startup safety review before highly hazardous chemicals are introduced.
We carry out an independent Stage 3 assessment after installation and before operation, resolve the findings and hand over to the Phase 6 operations team.
Element implementation flow chart
A decision gated workflow that shows the actual sequence of activities from initiation through steady state operation, with key decision points highlighted.
What we produce
- Factory and site acceptance test procedures with their execution records
- End to end validation evidence for each function
- A Stage 3 functional safety assessment closure pack
- A clause 15 validation report with the as left setpoints and response times
- A punch list with the deviation resolution closed out
- A pre startup safety review sign off record to paragraph i of OSHA PSM
Where execution fails
- A factory acceptance test that skips voting and bypass combinations
- A site acceptance test run as loop checks rather than end to end actuation
- Validation evidence missing for the legacy field elements
- Response time validated only to the logic output rather than full closure from sensor to valve
